    Service organizations increasingly organize themselves and operate on a value chain level. This creates important challenges and opportunities, which call for a realignment of strategic focuses, in particular with respect to the impact of technovation on service creation and services modus operandi, their resulting service classification, and the restructuring amongst different service value chain industries. This research builds on a recently developed classification scheme, referred to as the Services Cubicle, that transcends current industry boundaries and includes upcoming service business trends in technovation. The paper subsequently illustrates a variety of service industry examples in order to clarify the resulting service classifications, taking into account deployment of varying degrees of technovation in that industry

    Opportunities and Pitfalls Associated with Coordination Structures in Supply Chain Management: An Exploratory Case Study

    Supply chain management has recently received considerable attention with an aim to reduce production costs, manage risks, reduce delays, maximize profit, and improve the quality of products, with the result of increased competitiveness and profitability for all stakeholders. It is in this context that this paper investigates what coordination structures (focal, mediated or collaborative) are adopted by supply chains in an attempt to conduct integrative planning. We investigate what coordination structure is required by the focal firm to successfully manage supply chain activities. This paper draws on evidence from the extant literature and demonstrates the prevalence of the newly defined coordination structures in the manufacturing of complex products with a multitude supply chain based on findings from exploratory case studies. Potential opportunities and pitfalls associated with each of the coordination structures in regards to governance, supply risk, quality and supplier involvement practices are examined. Hypotheses are developed to help understand the impact of coordination structures on various supply chain activities. Previous studies have not considered the potential pitfalls and opportunities for the focal firm in choosing to adopt a particular coordination structure in the complex products manufacturing industry, which poses specific regulatory considerations. The two explanatory case studies also consider the perspective of tier 2 suppliers, which are not commonly considered in the supply chain literature. Moreover, this research is able to demonstrate that there is no such coordination structure as one size fits all, and instead illustrates that even different component supply chains in the same organisation can have different coordination structures

    Prophylaxis of post-operative nausea and vomiting in major obstetrics surgeries: a comparative study between palanosetron and neit guam point stimulation

    Background: Post-operative nausea and vomiting (PONV) prophylaxis is an important component in providing safe anesthesia in antenatal women. Pharmaceutical interventions like metoclopramide and 5-HT3 antagonists are the current treatment of choice for PONV prophylaxis. However newer drugs are less easily available and more expensive and there is always a concern regarding the effects on fetus. The objective of the study was to evaluate and compare efficacy and safety of antiemetic effects of stimulation of neitguam point stimulation with palonosetron for PONV prevention in obstetric surgeries.Methods: A randomized control trial was done on 150 patients with ASA grade I-II, between ages of 18-45 undergoing obstetric surgery under spinal anesthesia at a tertiary care center. The patients were randomly allocated into two groups using a random sequence. In group I, 76 patients received 75μg palonosetron IV 5 minutes prior to induction. Group II contained 74 patients in which neitguam point stimulation was done by acupressure (wrist band) . All patients were followed till 24 hours post-surgery and incidence of PONV was recorded intra-operative and till 30 minutes, at 2 hours, 6 hours and 24 hours post-surgery. The results were analyzed using SPSS software and chi square test.Results: Both neit guam point stimulation and palanosetron were effective in prevention of PONV (p=0.32). neit guam point stimulation can be considered as effective as palonosetron in prevention of nausea. The cost analysis signifies the advantage of neit guam point stimulation.Conclusions: Neit guam point stimulation is a safe and highly effective method to prevent PONV. It can help in bringing down cost and can be easily made at the small primary healthcare centers and trained midwives

Symptomatic leiomyomas are managed with either medical therapy or surgical management in the form of myomectomy or hysterectomy. We are reporting an interesting and rare case of a large submucous fibroid which was spontaneously expelled per vaginally during clinical examination

    Synthesis and Characterization of nZVI Grafted Alumina and Its Application for Fluoride Removal from Drinking Water: Equilibrium and Kinetics Study

    In the present study attempt was made to synthesize iron nanoparticles in the presence of alumina (Al-nZVI) by sodium borohydride reduction process. The composite adsorbent has been characterized using various analytical techniques such as scanning electron microscopy, Fourier transform infrared spectroscopy, X-ray diffraction and transmission electron microscopy which showed that iron nanoparticles were partially dispersed on alumina surface, with their diameter being in the range 40–100 nm. The batch adsorption experiments were carried out to study the effect of different parameters. The maximum removal of fluoride was obtained at optimal condition of pH 5.0 and dose = 1.8 g/L and was observed to be 94% with contact time of 60 min at 40 °C and initial concentration of 2 mg/L. The adsorption data fitted that pseudo second order kinetics and followed Langmuir isotherm model with maximum adsorption capacity of 10.06 mg/g. Thermodynamic study revealed exothermic nature of adsorption

    Objective: Present study ascertains the neuroprotective potential of curcumin in olfactory lobes, cerebrum, hypothalamus-hippocampus, cerebellum and pons-medulla of mice, intoxicated with lindane.Methods: For the study, mice were divided into four groups. Olive oil was given as a vehicle to the mice of group I. Mice belonging to groups II and III was administered with lindane and curcumin respectively, for 12 h exposure by intraperitoneal injection. In group IV, curcumin was administered 10-15 min prior to exposure of lindane.Results: Lindane exposure significantly increased the activities of thiobarbituric acid reactive substance (TBARS) (p<0.05) and protein carbonyl content (PCC) (p<0.05) whereas decreased the activity of reduced glutathione (GSH) (p<0.05) and superoxide dismutase (SOD) (p<0.05). Treatment of curcumin alone as well as in combination with lindane significantly declined the level of TBARS and PCC (p<0.05) and increased the activity of GSH and SOD (p<0.05).Conclusion: Curcumin has neuroprotective potential. It can be used as a therapeutic agent against lindane induced toxicity.Keywords: Lindane, Curcumin, Oxidative stress, Endogenous antioxidan

    Study of knowledge, attitude, practices regarding PPIUCD among antenatal women at a tertiary care centre in Northern India

    Background: PPIUCD has been introduced in the national family welfare program since March 2010 in several states. AN IUCD can be inserted in 48 hours postpartum, referred to here as postpartum intrauterine contraceptive device. This study was done to assess the Knowledge, attitude and practice of postpartum Intrauterine contraceptive device in antenatal patients at a tertiary care centre in Northern India.Methods: This is a questionnaire based cross sectional observational study including 350 antenatal women attending antenatal OPD over a period of 6 months.Results: Out of these 350 women, 126 women (36%) had knowledge of PPIUCD. only 30 % of women had previous knowledge about PPIUCD however only 10 % of women practiced it in the past. After appropriate counselling 18% of women agreed for insertion of PPIUCD after this delivery.Conclusions: The study concludes that the antenatal women had poor knowledge regarding PPIUCD. Practices were even worse. This might be attributed to low education ,  refusal by family especially male partner, and religious beliefs. But once appropriate knowledge and information is provided, attitude gradually changes

    Visual inspection with acetic acid for cervical cancer screening in a tertiary health care centre

    Background: Cervical cancer is one of the most prevalent malignant neoplasms among women in developing countries. Invasive cervical cancer is preceded by a long premalignant phase known as cervical intraepithelial neoplasia (CIN). The goal of cervical cancer screening is the detection and treatment of precancereous lesions before cancer develops. The objective of the study was to assess visual inspection with acetic acid (VIA) as a screening tool for use in a well-equipped health center , to evaluate VIA as an alternative or adjunct to the Papanicolaou (Pap) smear, and to determine if VIA can play a role in settings other than low resource ones.Methods: This was a prospective study of 1520 asymptomatic women, carried out in 2014-2015. The study was performed at a tertiary care center equipped with the latest-generation technology and highly trained oncologists. The women underwent a complete clinical evaluation, including a Pap smear and VIA. Participants with any positive test were referred for colposcopy and biopsy.Results: More women tested positive by VIA than on the Pap smear (6.9% vs. 4.2%; P =0.0001). There were 27 women with histologic cervical intraepithelial neoplasia grade 1 (CIN1); of these, 12 were detected by Pap and 15 by VIA (P = 0.4). A diagnosis of CIN 2 or 3 (CIN2–3) was confirmed in a total of 11 cases; Pap detected 4 of the cases and VIA 9 of the cases (P = 0.06). The positive predictive value for detection of CIN 2+ was 8.5% for VIA and 6.2% for Pap (P = 0.5). Most importantly, while only 2.8% of patients with a positive VIA were lost to follow-up before colposcopy that was true for 26.5% of the women with a positive Pap smear (P < 0.0001).Conclusions: VIA is useful for detection of precursor lesions of cervical cancer not only in low-resource settings but also in well-equipped health centers and cancer centers. In these non low-resource settings, VIA has a positive predictive value comparable to the conventional Pap smear, but it is more likely to achieve earlier diagnosis, follow-up, and treatment than cytology based screening

    The anti-inflammatory effects of Curcuma longa and Berberis aristata in endotoxin-induced uveitis in rabbits

    Purpose. To investigate the anti-inflammatory effect of topical application of Curcuma longa (C. longa) and Berberis aristata (B. aristata) aqueous extracts on experimental uveitis in the rabbit. Methods. Anterior uveitis was induced in rabbits by intravitreal injection of lipopolysaccharide from Escherichia coli after pretreatment with C. longa and B. aristata aqueous extracts. Subsequently, the anti-inflammatory activity of C. longa and B. aristata was evaluated by grading the clinical signs and histopathologic changes and estimating the inflammatory cell count, protein, and TNF-α levels in the aqueous humor. Results. The anterior segment inflammation in the control group was significantly higher than in both the extract-treated groups, as observed by clinical and histopathologic grading. The inflammatory cell count in the control group was 30.75 ± 7.33 &#215; 105 cells/mL, whereas it was 2.39 ± 0.59 &#215; 105 (P &lt; 0.001 vs. control) and 11.56 ± 2.44 &#215; 105 (P = 0.001 vs. control) cells/mL in the C. longa– and B. aristata–treated groups, respectively. The protein content of the aqueous humor was 18.14 ± 4.98, 3.16 ± 0.55 (P &lt; 0.001 vs. control), and 8.24 ± 1.42 (P &lt; 0.01 vs. control) mg/mL in the control, C. longa–, and B. aristata–treated groups, respectively. The aqueous TNF-α level in the control group was 976.29 ± 66.38 pg/mL and was 311.96 ± 28.50 (P &lt; 0.0001 vs. control) and 654.09 ± 47.66 (P &lt; 0.001vs. control) pg/mL in the C. longa– and B. aristata–treated groups, respectively. Conclusions. Topical instillation of aqueous extracts of C. longa and B. aristata showed potent anti-inflammatory activity against endotoxin-induced uveitis in rabbits
